- Please complete the application form (PDF)(Word) including:
- Annex 1: Information About the New PRH Estate(s) (if applicable);
- Annex 2: Proposed Income and Expenditure Breakdown (Excel);
- Annex 3: Implementation Plan and Expected Output of Intervention Strategies (Excel);
- Annex 4: Disclosure of Conflict of Interests with organisation chop and the signature of Chairperson or Head of your organisation (and the joint applicant organisation(s), if any);
- Annex 5: Details of Experience in Organising/ Assisting in the Organisation of Activities/ Programmes Echoing/ Supporting Government Policies/ Measures;
(Please ensure Organisation chop and the signature of Chairperson or Head of your organisation (and the joint applicant organisation(s), if any) are on the declaration in section 6 of the application form.)
- Copies of organisation registration document (including the joint applicant organisation(s), if any) under the relevant ordinance(s);
- A set of the most recent audited reports or certified management accounts (i.e. within one year); for management accounts, please include (1) Income and Expenditure Accounts/Income Statement and (2) Balance Sheet/Statement of Financial Positions;
- Independent research proposal (if applying for research and evaluation expenses); and
- Attachments or supplementary information of the Project (if any).

Please mail or personally deliver the completed application form (one original and two copies) with supporting documents (three copies each), and a USB drive containing all electronic files to:

Please note that duplicate application, using of non-designated application form, late applications and applications submitted by email or by fax will not be considered.
For applications submitted by post, applicant organisations should ensure that sufficient postage is affixed before posting. The postmark date on the envelope shall be the date of application.
Electronic Application
- Before using this system for electronic application, please read carefully the "General FAQs”.
- Before submitting the electronic application, please complete and have the following documents ready for upload.
- Please check whether the information entered is correct. Once the electronic application is submitted, all the information will no longer be available for editing or viewing. If necessary, applicants should print/save the application immediately after submitting the electronic application.
- To receive an auto-generated confirmation of submission for record, please fill out your email address in the electronic application form. The system will confirm your submission by email within two hours.
- After submitting the electronic application, please keep the original copies of the application form and relevant documents.

Result of Applications
Provided that all the information received are complete and accurate, the applicants would be informed of the results in due course.

Signing the Conditions of Grant (CoG) and Project Commencement
All applicants of approved project have to sign the Conditions of Grant (CoG) and commence the project within specified timeframe; otherwise it will be regarded as lapsed. However, special consideration could be granted on individual merits.
