Critical Success Factor

Hong Kong East Cluster played a leading role in linking up Pamela Youde Nethersole Eastern Hospital (PYNEH) and Tung Wah East Hospital, aiming at building up social capital for the long-term wellness of chronic patients, psychiatric patients and their care-givers. To strengthen information sharing, Hong Kong East Cluster also set up a professional health care team, regularly attend meeting with the project team.

Hong Kong East Cluster of the Hospital Authority has been committed to developing "Medical-Social Collaboration" for years. It is our strong belief that not only the medical and nursing care, but also the psychosocial support and the community resources do help patients and their care-givers for better treatment and rehabilitation along patient's journey and sustaining their good quality of life. Therefore, Community & Patient Resource Department of Pamela Youde Nethersole Eastern Hospital (PYNEH) has built up seven platforms by engaging clinical departments in our cluster as well as community partners so as to strengthen mutual communication and collaboration through referral system and service integration.

As a strategic partner of two projects, "SMARTCare Movement - Caring Community for Carers" project launched by Community Rehabilitation Network and "Family Networks at Community" jointly organised by Baptist Oi Kwan Social Service and Richmond Fellowship of Hong Kong, Community & Patient Resource Department played a leading role in linking up Department of Medicine and Department of Psychiatry of PYNEH in the early stage and later Tung Wah East Hospital as well as important stake-holders of different sectors in the community, aiming at building up social capital for the long-term wellness of chronic patients, psychiatric patients and their care-givers. Medical-Social Collaboration has been widely expanded and consolidated. "Care Networks" are forged and rooted in Hong Kong East.

We deeply thank the Community Investment and Inclusion fund for the Outstanding Social Capital Partnership Award (Corporate). It is a great honour and due recognition to assure our contribution to Medical-Social Collaboration and make our mission "to build up more useful community resources for our patients" a big step forward.
