Critical Success Factor

With great support from the hospital, the project has been the first one to apply the medical-welfare-community collaboration model. The hospital has not only arranged community nurses and hospital volunteers to participate in the project, but also attended meetings regularly with the project to give valuable advice in organising, implementing and reviewing the project. For many years, the hospital has all along been maintaining close co-operation with the project agency and adopting the "one estate one nurse" intervention model in four more CIIF projects covering six public estates in Kwai Tsing district. Making use of medical records in the hospital, the community nurses have been able to provide accurate health assessment and proper medical advice for elderly residents. It is found that the project has not only helped reduce medical cost effectively, but also demonstrated the valuable experience of promoting primary medical care and ageing in place.
