1. Objectives
- To establish a community care network to enable the elderly to live in a safely and healthy in community.
- To engage the elderly in an active living with the neighborhoods in different groups.
- To strengthen and encourage the elderly to participate community activities and becoming productive aging.
2. Strategies
- To build up the existing community care network through the Kwai Tsing Safe Community and Healthy City in Lai King, including the Integrative Community Health Center, Family Medicine and Community Nursing to focus on the care of vulnerable with chronic illness and hidden elderly.
- To set volunteer visiting teams in each block to provide resources and information to vulnerable elderly; to identify the potential resources from the elders and strengthen them to help others.
- To collaborate NGO, Government department, private enterprise , estate management, District Councillor and local schools, families to build up the platform to help the elders to establish mutual support network and living safely in Lai King.
3. Expected Outcomes and Performance Indicators
- An Estate Intranet for elderly is built through collaboration with district Councillor, Private enterprise, Volunteers, local schools and hospitals, NGOs and estate management
- Different group volunteer teams are set up to support the needy elderly and provide regular home visits to identify the risk
- Empower the elders to be self- help and help others by participating the community activities
4. Brief Description of Programmes
- South Kwai Chung Service Centre is the resources and service coordination center for the elderly residents. It will provide nurse clinic, health assessment, health education, volunteer training and social activities for elders .Volunteers will be trained to making home visits to the vulnerable groups. With the assistance of the district Councillor, NGOs, local volunteers, a caring network can be built among the residents. To support and empower the elders to help the ones in need by conducting community programes for them
5. Unique Feature and Areas of Innovation
- The Project is to cut across the traditional service boundaries. It aims to mobilize the local community resources to strengthen the elders become the assets of neighborhood to be self- help and help others. The vulnerable elderly who cannot reach out can be cared by local supportive neighborhood networks so that the quality of life can be improved
